There is a $15.00 administration fee per child for all non-Iowa Medicaid children receiving vaccinations through our immunization clinics.  We accept cash, check, credit or debit cards for payment at this time. In addition, we are unable to accept children with Hawk-I coverage.  These children should be taken to their physician for immunizations.

Notice Regarding Public Hearing - Siouxland District Board of Health

The Siouxland District Board of Health will hold a reading of proposed Rule 23-4 to provide for the adoption of Iowa Administrative Code Chapters 567-38, 567-39, 567-49 and 567-69 which provides regulations for private water well permitting, nonpublic water well construction, well-plugging, and private wastewater disposal systems in order to protect the groundwater in Woodbury County and to meet the minimum standards for ground water protection in Iowa on Wednesday, January 12, 2022, at 12:15 p.m.

Said hearing shall be held at the Siouxland District Health Department, 1014 Nebraska Street, Sioux City, Iowa, in the Board Room located on the second floor. Copies of said item may now be examined in the Administrative Offices in said Health Department by any interested persons and available online at

All persons who wish to be heard in respect to this matter should appear at the aforesaid hearing.

Siouxland District Health Department is currently scheduling appointments for first series as well as booster doses for the following types of vaccine: 

  • Pfizer (ages 5 and older)
  • Moderna (ages 18 and older)
  • Johnson & Johnson - single dose (ages 18 and older)

  • If you need to cancel or reschedule your COVID-19 vaccination, click on the link provided in your appointment confirmation email.
  • If you have health questions or concerns regarding the vaccine or are wondering about vaccination after recent COVID infection, please consult your health care provider.

Move Your Way CampaignSM

Siouxland District Health Department has been selected as one of three communities from across the nation, to pilot the Move Your Way Campaign SM over the next nine months. The U.S. Department of Health and Human Services created the Move Your Way campaign to promote the second edition of the Physical Activity Guidelines for Americans. It aims to help people understand how much physical activity they need to be healthy and how to fit it into their daily lives.

The campaign has videos, fact sheets, posters, and interactive tools in English and Spanish that are free and easy to use and distribute. These materials promote friendly, practical key messages, like “lots of things count as physical activity” and “it all adds up!”
