IMMUNIZATIONS

Need a copy of your child's immunization record? Click Here. Must have name, date of birth, and a Social Security or Medicaid number.

To schedule an appointment, call 712-279-6119 and ask for the clinic.

All clinics held at SDHD, 1014 Nebraska St.

There is a $15.00 administration fee per child for all non-Iowa Medicaid children receiving vaccinations through our immunization clinics.  We accept cash, check, credit or debit cards for payment at this time. In addition, we are unable to accept children with Hawk-I coverage.  These children should be taken to their physician for immunizations.

COVID-19 Vaccination Locations

    • is currently scheduling appointments for the first and second doses for the following types of vaccine: 
      • Pfizer (ages 12 through 17)
      • Moderna (ages 18 and older)
      • Johnson & Johnson - single dose (ages 18 and older)
    • Schedule your appointment online or call 712-279-6119. 
    • If you need to cancel or reschedule your COVID-19 vaccination, click on the link provided in your appointment confirmation email.
    • If you have health questions or concerns regarding the vaccine or are wondering about vaccination after recent COVID infection, please consult your health care provider.

Siouxland District Health Department Services During COVID-19 Disaster Declaration

Most services at SDHD have resumed as normal.  At this time, individuals that are not fully vaccinated for COVID-19 are requested to wear a mask when entering our building for services. 

  


Move Your Way CampaignSM

Siouxland District Health Department has been selected as one of eight communities from across the nation, to pilot the Move Your Way Campaign SM over the next nine months. The U.S. Department of Health and Human Services created the Move Your Way campaign to promote the second edition of the Physical Activity Guidelines for Americans. It aims to help people understand how much physical activity they need to be healthy and how to fit it into their daily lives.

The campaign has videos, fact sheets, posters, and interactive tools in English and Spanish that are free and easy to use and distribute. These materials promote friendly, practical key messages, like “lots of things count as physical activity” and “it all adds up!”
