IMMUNIZATIONS

Need a copy of your child's immunization record? Click Here. Must have name, date of birth, and a Social Security or Medicaid number.

To schedule an appointment, call 712-279-6119 and ask for the clinic.

There is a $15.00 administration fee per child for all non-Iowa Medicaid children receiving vaccinations through our immunization clinics.  We accept cash, check, credit or debit cards for payment at this time. In addition, we are unable to accept children with Hawk-I coverage.  These children should be taken to their physician for immunizations.

Tyson Event Center Vaccination Clinic Appointment Information PDF Print E-mail

If you are registered for a vaccination clinic at the Tyson Event Center, this page includes details about attending the clinic including a map of the area, instructions about where to park, what to bring, and options for assistance.  Pre-scheduled appointments are required for these clinics.

Enter the parking lot from either Pierce or Pearl Streets.

Follow the posted signs.

Please do not arrive for your appointment early, or if you do please stay in your vehicle until within 5 minutes before your appointment time. Due to space constraints and for social distancing purposes, we will not have a waiting area available.

Those not requiring assistance are encouraged to park in the area directly south of the main entrance. 

Enter the Tyson Center through the Box Office doors on the south side of the building.

Only those with an appointment should come inside the clinic, with the exception of those needing assistance during the clinic. If you need someone to assist you it is fine to bring a person with you.   

Wheelchairs will be available for those needing them. 

Interpreters via the language line will be available.

The Prevaccination Checklist will be available at your appointment in English, Spanish, Vietnamese, and large print or you may print it, complete it at home, and bring it with you to your appointment. We also have large magnifiers available for use and volunteers will be available to provide assistance with completion of the form. 

Masks are required to be worn throughout the duration of your appointment. SDHD recognizes and understands that it may not be feasible for some individuals to wear a mask. Therefore, a person with a disability who cannot wear a mask, or cannot safely wear a mask, for reasons related to the disability, are exempt from the mask requirement. 

Please bring ID verifying that you are included in the priority group eligible to receive vaccine (ID showing age, work ID, pay stub, etc...) 

Please wear layers and loose clothing that allows easy access to your upper arm.

Plan for your appointment to last approximately 20-30 minutes.  Individuals must be monitored for at least 15 minutes after receiving the vaccine, but seating will be available during this time. 

You will be scheduled for your second dose appointment prior to leaving the clinic.

As a reminder, if you are feeling unwell or are running a fever of 100.4 degrees F or higher, we ask that you please cancel or reschedule your appointment. You can use the link provided at the bottom of your confirmation or reminder message.
