There is a $15.00 administration fee per child for all non-Iowa Medicaid children receiving vaccinations through our immunization clinics.  We accept cash, check, credit or debit cards for payment at this time. In addition, we are unable to accept children with Hawk-I coverage.  These children should be taken to their physician for immunizations.

Are you a Dental Hygienist(RDH) looking for a new challenge and enjoy working with children? Family friendly work environment, with normal hours Monday-Friday 8:00-4:30. Strong interest in the health of pediatric, adolescent and maternal health clients. Experience in oral health, maternal/child health, public health and office/clinic settings is encouraged. Must possess a Dental Hygienist license in Iowa (or able to obtain one upon hire). Must have a Bachelor’s Degree in a health-related field.

The Public Health Nurse Clinic Manager is responsible for the leadership of daily operations of nursing services and supervision of staff as a member of a clinic management team.  Communication skills and the ability to assure that clinic operations are maintained by updating and writing policy is necessary.  Development of clinic reports on services and quality improvement work.  Maintains a cooperative relationship among healthcare teams by responding to requests and participating in problem-solving methods.  Maintains professional knowledge in public health nursing practice.

Responsible to provide health and program specific information to clients seeking assistance and services from Siouxland District Health Department.  Will perform designated duties of assigned programs to targeted clients.  Duties may include working in multiple public health programs, data collection and entry, monitoring and ordering program supplies, enrolling/registering and screening clients, maintenance of client records, and entry of client data in program databases.  This position reports to Clinic Intake Manager.

Responsible and accountable to provide a broad range of services to clients in the home setting according to policies and procedures of Siouxland District Health Department, with the goal of achieving and/or maintaining client independence and to reduce, prevent or delay inappropriate institutionalization of clients.  Direct Care Workers are trained and supervised individuals who provide services, care and emotional support to clients.  They work in the Personal Care Only/Homemaker Program (PCO/HM) serving Woodbury County clients in their homes Monday through Friday 8:00-4:30pm. Reports directly to and responsible to the Personal Care Only/Homemaker Program (PCO/HM) Program Manager, and the Nursing Director.

Join a team of Public Health Nurses providing health services and education to Woodbury County Residents that focus on the prevention or spread of diseases.  Services are provided daytime Monday through Friday at Siouxland District Health Department (SDHD) in Sioux City, IA.  SDHD offers a wide array of services in a progressive public health environment that allows you the ability to interact with professionals working in our family and community health, acute infectious disease EPI and preparedness, environmental and health promotion and chronic disease prevention arenas.

Responsible and accountable to provide eligible families the Healthy Families America (Healthy Opportunities for Parents to Experience Success – HOPES) program under the supervision of HOPES Program Manager/Supervisor.  Activities include provision of in-home visits, communication with families, hospital staff, physicians and community agency partners, family education, family assessment, development of family goal plan, assessment of infant and child development, linking families to community services, and collection of family data
