There is a $15.00 administration fee per child for all non-Iowa Medicaid children receiving vaccinations through our immunization clinics. We accept cash, check, credit or debit cards for payment at this time. In addition, we are unable to accept children with Hawk-I coverage. These children should be taken to their physician for immunizations.

Application for a permit to install a private water well in Woodbury County. This is a fillable form that you can complete, then print and turn in to SDHD with the appropriate fee. Forms cannot be submitted electronically. Please note that a permit from the City of Sioux City indicating approval to install a private well within the City of Sioux City limits will also be required before a permit will be issued from this office. Form to waive the Time of Transfer inspection requirement for a septic system. The future installation form is used when it is assumed that a septic inspection will fail the Time of Transfer inspection, and it is agreed to install a new system instead of undergoing inspection. A Time of Transfer inspection, or a waiver of the inspection, must be completed and submitted at the time of sale in order to record the transfer of the deed to the property. This form should NOT be used if an inspection has been completed, but the system failed, as the inspection meets the recording requirements. Form to waive the Time of Transfer inspection requirement for a septic system. The future inspection form is used when a septic inspection can not be completed prior to the sale of a property due to factors such as weather conditions. A Time of Transfer inspection, or a waiver of the inspection, must be completed and submitted at the time of sale in order to record the transfer of the deed to the property.
