Need a copy of your child's immunization record? Click Here. Must have name, date of birth, and a Social Security or Medicaid number.

There is a $15.00 administration fee per child for all non-Iowa Medicaid children receiving vaccinations through our immunization clinics.  We accept cash, check, credit or debit cards for payment at this time. In addition, we are unable to accept children with Hawk-I coverage.  These children should be taken to their physician for immunizations.

Tobacco Free Siouxland

Tobacco Free Siouxland is a coalition organized by community agencies and partners to provide tobacco education, prevention and cessation resources to residents of Siouxland.

There are a variety of smoke free housing options in Woodbury County. View a list of all smoke free housing properties or for more information on becoming a smoke free apartment, visit the Iowa Department of Public Health Department's Smoke Free Homes found here.

Tobacco Free Worksites

Worksites are encouraged to evaluate their tobacco policies. Many worksites are enacting policies that restrict tobacco use on the grounds, in parking lots and even during the employees shift. Siouxland District Board of Health acknowledgements 

The Siouxland District Board of Health signs letter of acknowledgement to WinnaVegas Casino Resort for their public health efforts adopting an indoor smoke-free policy for the facility. Find a copy of the letter here.
